Risk Evaluation and Patient Safety Strategies 

Venous Thrombosis (VTE), which is the disease process of deep vein thrombosis (DVT) and or pulmonary embolism (PE), is one of the most patient risks, that can be prevented if special precautions are taken. VTE can affect anyone, many of them occur during hospitalization or soon after discharge. “A study by the National Hospital Discharge Survey (NHDS) showed that between 2007 to 2009, nearly 500,000 patients that had been discharged developed VTE”. (CDC: Centers for Disease Control, 2020). This paper will discuss patient safety strategies, literature review, contributing factors, risk theories, and Quality and Safety Education for Nurses (QSEN).
