In your initial response, please describe the concern. Does the concern primarily occur at the micro, meso, or macro level? How would you address this issue? SOLUTION 

A concern that I have for my future area of practice is potentially contributing to the opioid epidemic in a negative way. In my home state of Maryland, nurse practitioners have unrestricted prescriptive authority. This new responsibility and autonomy is quite an exciting thing, however, it does provoke some apprehension. Working in intercity Baltimore, I have seen firsthand the impact that the opioid use disorder has on individuals and on communities. A great worry of mine is struggling to know the appropriate amounts of opioids to prescribe without creating dependence—especially as a novice nurse practitioner. I do not want to prescribe them too freely, but I also want to make sure I am controlling my patients’ pain appropriately. Policies also vary from state to state and from facility to facility, so in the event that I want to move and practice elsewhere, I need to be mindful about how this would affect my prescriptive authority.
