SUBJECTIVE DATA
Chief Complaint (CC): Chest Pain
History of Present Illness (HPI): Mr. Brian Foster is a 58-year-old caucasian male who came to the shadow health clinic for chest pain presented in three different episodes last month, which is now happening very often. The patient described the pain as tightness, rating it as five out of ten in severity, which lasted a couple of minutes and is located in the middle of the chest. Brian reports that physical activities such as gardening and climbing the stairs aggravate the chest pain with no associated symptoms. He said not taking any medication to relieve the ache because resting helps. The patient denies fever, cough, chills, nausea, and headache; he further denies smoking or using illicit drugs however drinks two to three beers during the weekend.
Medications: Lisinopril 20mg P.O. daily; Atorvastatin 20 mg P.O. daily at bedtime; Omega-3fish oil 1200mg P.O. twice daily.……..please click the icon below to purchase full answer at $10
