Part I: Socioenvironmental health is part of the comprehensive geriatric assessment (CGA) and is a component that assesses social isolation among the elderly. According to Kennedy-Malone et al., (2019) social isolation is a leading factor in mortality among the elderly population.

 

Part II:

The last component of the CGA assessment is Quality of life in aging individuals. This evaluates the elderly general satisfaction with life in the domains identified (Kennedy-Malone et al., 2019). It also addresses current health care decisions and end of life care. Patients are encouraged to obtain advance directive to spell out their wishes.

Lubben Social Network Scale—6-item version

This tool is used to assess relationship with friends and family; it evaluates if the patient may be at risk for social isolation (Kennedy-Malone et al., 2019).

 

Components: Consist of the following questions
