SOLUTION 

Transitioning patients from hospital to another hospital, or from hospital to home, or from hospital to a rehabilitation facility requires collaboration among different healthcare professionals so as to ensure that the needs of the patient and those of their families are taken into consideration, and everything is done for the best interest of the patient. Transition care is important in healthcare because it defines a broad range of time-limited services that are designed to ensure that there is healthcare continuity, promote the safe and timely transfer of patients, and to avoid preventable poor outcomes among at-risk populations (Melby, Brattheim and Hellesø, 2015).

My facility promotes inter-professional collaboration during patient transitions by defining the roles of every healthcare professional during the process.
