SOLUTION
iCare Assignment – Components of iCare
Acute clinical nurses conduct dialysis medications for patients in healthcare settings. In this regard, these nurses use primary team care model that permit them to offer optimum quality care to their patients with a considerate level of care and autonomy. As part of healthcare professionals and caregivers, nurses collaborate with nephrologists, primary care nurses, and other interdisciplinary teams in offering the best quality care to optimize clinical results.
Furthermore, offering care based on resilience, compassion, advocacy, and founded on evidence- based practice (EBP) will improve collaboration with others and highlight patient-centered care. One of the perfections that nurse practitioners can attain in their clinical setting is the capability to contribute in helping patients in the process of transition to a chronic outpatient setting from acute hospital settings as well as continuity of care and treatments.

It is not only related to the clinical outcome and satisfaction of the patient but also improves alliance with other interprofessional multidisciplinary groups. This creates synchronization of the flow of work and safe work surrounding.
