SOLUTION 

In this week’s discussion we are identifying a challenging process that we encountered personal or professional. I have chosen to choose one from my professional experience I have through out my career in nursing have been in nursing management roles in the long-term care setting. There have been many processes that needed changing one of my signature achievements as a director of nursing was instituting a falls management program aimed at the reduction of facility falls. The identified issues were a lack of documentation after a fall, interventions to prevent falls, and post fall intervention implementation by the nursing staff. The nursing staff was seemingly set in the way they did things and of course this made making changes while necessary much more difficult, this change was identified and was a systematic planned change. This change had many elements that made it difficult to implement because the nurse’s perception of the change was that the facility and management were just making more work for them. This change required re-educating the staff on new fall protocols, interventions, and documentation practices to maintain patient safety prior to and after a fall.
